CMS's Million Hearts® Model Longitudinal ASCVD Risk Assessment Tool utilizes the 2013 pooled cohort equation created by the ACC/AHA to calculate ASCVD risk as the foundation for calculating baseline ASCVD risk and updated 10-year ASCVD risk for primary prevention patients who have initiated and are complying with therapy (i.e., aspirin, statin, blood pressure control, smoking cessation) alone, or in combination. The tool also supports provider and patient shared decision making related to the initiation of therapies.
